Getting a financial assessment

If you're moving into a residential or nursing home, or you're getting care at home, we'll make sure you only pay as much as you can afford for your care.

To find out how much you can afford, we'll do a financial assessment.

You can apply for a financial assessment using our online form.

As part of the assessment, you'll need to provide evidence. You can attach it to the online form. You can take photos of your evidence, as long as we can read everything on the document.

This page will tell you about the evidence you’ll need to send, so you can prepare everything before you start the form.

We strongly recommend that you sign up for a MyMiddlesbrough account first. This will let you save the form and come back to it later if you need to.

Please note, when we say 'you' on this page, we mean the person who is getting care. If you're filling in the form on behalf of someone else, we will refer to them as the 'client' on the form.

If you're acting on someone's behalf

If you're filling in the form on someone else's behalf, you'll need to prove that you have permission.

You'll need to show evidence that you:

  • are their appointee - you'll need to upload a copy of a letter from the Department for Work and Pensions
  • have lasting or enduring power of attorney - you'll need to upload a copy of the document confirming power of attorney
  • are managing their affairs in another way - you'll need to explain what this is and upload a copy of a letter confirming that they're happy with this arrangement

Who to invoice

If you have to pay towards the cost of your care, we'll send you invoices so you can make the payments.

If you'd rather we sent the invoices to someone else so they can pay, you'll need to tell us their full name and address.

Your care home

If you're in a care home, we'll need to know the full name and address of the home.

If you've been admitted to hospital in the last 6 weeks, we'll need to know the dates you were admitted and discharged. If you've been in hospital more than once in the last 6 weeks, we'll need the dates of every stay.

Property or land you own

If you own your own home, we'll need to know:

  • if the property is currently up for sale, occupied, or rented to someone
  • if anyone else has part ownership, we'll need their full name and the percentage they own

If you own any other property or land, we'll need to know:

  • the address
  • the value
  • how much of the mortgage is left

If the property is currently rented to someone, we’ll need to know:

  • how much net income you're getting
  • how often the rent is paid (for example, weekly, monthly, etc.)

Your income

If you're working or getting benefits, we'll need to know:

  • how much you're getting
  • how often it's paid (for example, weekly, four-weekly, etc.)

If you have a partner, we'll also need to know this information for them too.

Your pension

If you're getting a pension (private pension, occupational pension, or any other pension), we’ll need to know:

  • how much you're getting
  • how often it’s paid (for example, weekly, four-weekly, etc.)

If you have a partner, we'll also need to know this information for them too.

Your savings and investments

Transfer of property, land, or savings

If you've transferred, disposed of, or gifted any property, savings, or capital worth over £10,000 in the last 10 years, we'll need to know:

  • the date it was transferred
  • the name of the person it was transferred to
  • the amount transferred

We'll need evidence as either a bank statement or a letter from the person who received the transfer.

Bank, building society, savings, and Post Office accounts

We'll need to see the last 2 months' bank statements for every bank, building society, savings, and Post Office account you have.

If you have a partner, we'll also need to know this information for them too.

Stocks and shares, ISAs, unit trusts, and investment bonds

If you have stocks and shares, ISAs, unit trusts, and investment bonds (with or without assurance attached), we'll need to see evidence of:

  • who the investment belongs to
  • the company it's invested with
  • the amount invested

If you have a partner, we'll also need to know this information for them too.

Personal injury awards

If you've received a personal injury award payment, we'll need to know:

  • how much you got
  • the date you got the payment

If you have a partner, we'll also need to know this information for them too.

Your outgoings

We'll need to know if you're paying:

  • rent
  • mortgage
  • Council Tax
  • service charge
  • child support or child maintenance
  • water rates
  • building insurance (not contents insurance)

We'll need to know:

  • how much you're paying
  • how often you're paying it (for example, weekly, four-weekly, monthly, etc.)

If you have a partner, we'll also need to know this information for them too.

Extra costs due to your disability

If you're applying for help with home care costs, we'll look at any additional costs you have because of a disability. For example, if you're paying for:

  • a special diet
  • extra laundry and bedding
  • extra heating
  • gardening
  • cleaning
  • domestic help
  • community alarm
  • transport
  • disability-related equipment
  • social costs

We'll need to know:

  • how much you're paying
  • how often you're paying it (for example, weekly, four-weekly, monthly, etc.)

If you send back your assessment form by post or email, you'll need to include all of your evidence with it.
